We DO NOT provide bedside care for any patients. It’s important for you to know that Hospice Navigation Services is NOT a hospice agency, and is not affiliated with any hospice agency. Our team is unbiased and impartial, providing guidance to anyone anywhere in the U.S.
Anyone who needs information about hospice and end of life care can benefit from a Navigation session. Persons with serious illness and caregivers for someone with an end stage disease will find a Navigation session helpful.
Most people who need to access hospice care don’t know where to start. They need information and resources to help them make the right choices for their end of life care. Hospice Navigation Services can provide you with expert guidance that’s unbiased and personalized to your specific needs. Your Navigator is an experienced professional that you can trust.
There are many signs that might indicate it’s time to book a Navigation session. If your healthcare team has said there’s a terminal prognosis (6 months or less to live), or if treatment is no longer working, it’s the right time to get information about end of life care. If your person no longer wants to go to the hospital or has had a major decline in health, you should get information about hospice. If you simply feel that you need education and support about end of life care, it’s time to book a Navigation Session.
You can expect to be talking to an experienced hospice professional. We can discuss any topics related to end of life care. Clients often use a Navigation session to discuss advance care planning, the basics of hospice care and eligibility, and to troubleshoot their current hospice care. (in italics) Your Hospice Navigator will never provide legal, medical, or financial advice.
All sessions are conducted by phone or by video call. It’s your choice. All 60 Minute Navigation Sessions can include up to 3 participants. This is a great way for caregivers to receive information at the same time and get everybody on the same page.
It’s easy to book your session on our website. To access our schedule of available appointments, click on any of the Schedule Your Session links on the Hospice Navigation Services page, or click the button below.
It’s easy to pay for your Navigation Session. Payments are made on the website when your session is booked, and we accept all major credit cards. We do not currently take health insurance.
